PLEASE READ CAREFULLY
At the Awards Luncheon on Friday, September 25, an announcement was made giving you instructions as to how to obtain the Substitute Teacher Reimbursement Request Form. Here is the information provided:
- On Monday, October 5, you will receive an email with a link for an online evaluation survey.*
- Complete the evaluation survey.
- The online survey will close October 12. Be sure to complete it as soon as you receive the link.
- Soon after October 12, you will receive a second email with "The Substitute Request Form" and a "Certificate of Attendance" to use for your professional growth/development points.
- Print the Substitute Reimbursement Request Form, provide your name and email address, and forward it to the appropriate person in your school district.
- All forms are to be completed and submitted to me by the date listed on the form.
